THE BIG PICTURE

Does staying with the same medical device company actually pay off?

It’s a question worth asking in an industry where territories change, quotas increase, and compensation plans rarely stay the same forever.

This week, we’re examining a new angle from the MDR Compensation Index: how earnings differ based on tenure with the same employer.

We’re also covering Zimmer Biomet’s accelerating sales force specialization and newly finalized Medicare payment changes that could influence where procedures are performed in 2027.

The common thread? Your earning potential depends on more than how well you sell.

MDR Compensation Index infographic comparing medical device sales earnings by company tenure. Among respondents with 11–15 years at their employer, 85% earn $200K+ and 69% earn $300K+, highlighting the relationship between employer tenure and compensation based on voluntary anonymous survey submissions.

Does Company Loyalty Actually Pay?

Medical device sales is built on relationships. For many reps, staying with the same manufacturer means established accounts, recurring business, and a territory that becomes more productive over time.

But does that loyalty translate into higher earnings?

We analyzed the latest voluntary, anonymous submissions to the MDR Compensation Index, examining compensation based on years with the same employer.

Three Findings Worth Considering

1. Longer tenure correlates with higher earnings.

Among reps with 3–5 years at their company, 24% earn $300K+, compared with 69% of those with 11–15 years. However, experience also plays a role. Correlation doesn’t prove loyalty drives higher compensation.

2. High earners may have fewer reasons to leave.

Established surgeon relationships, recurring revenue, and mature territories create financial incentives to stay. For a rep earning $350K, switching manufacturers could mean sacrificing years of territory development.

3. Staying doesn’t guarantee continued growth.

Only 22% of respondents with 21+ years at their employer report earning $300K+. While that group includes just nine respondents, it raises an important question: Is your compensation growing alongside the value you’re creating?

Why It Matters for Reps

Loyalty alone shouldn’t determine your career decisions. If revenue and quotas keep increasing while compensation remains flat, it’s time to evaluate your options.

The goal isn’t to stay or leave. It’s to maximize your long-term earning potential.

Zimmer Biomet’s Sales Force Transformation Is Accelerating

More than 75% of Zimmer Biomet’s U.S. sales force is now dedicated to specific business segments.

During its November 4 earnings call, Zimmer provided an update on its ongoing commercial restructuring.

The company is three quarters into an eight-quarter transition toward a more specialized U.S. sales organization.

Management indicated the transformation is progressing ahead of expectations, with less disruption than initially anticipated.

Where Zimmer Is Investing

The company identified continued sales force investment across:

  • Upper extremities

  • Sports medicine

  • Craniomaxillofacial and thoracic

  • Robotics

  • Foot and ankle

Zimmer also reported U.S. technology and data sales growth exceeding 30% during the quarter.

Why It Matters for Reps

Specialization creates opportunities, but potentially new compensation challenges.

Dedicated product representatives may be better positioned to develop clinical expertise, pursue competitive conversions, and grow targeted categories.

But specialization can also mean dividing existing territories, redistributing product responsibilities, and changing commission structures.

For experienced reps, those changes could have meaningful financial consequences.

Imagine spending ten years building a profitable territory, only to have certain product lines reassigned to specialized representatives.

Does your quota decrease proportionally?

Do you retain commission credit?

Are new opportunities sufficient to replace the revenue?

These are questions reps should ask during any commercial restructuring, not claims about Zimmer’s specific compensation arrangements.

MDR Takeaway: Specialization may create attractive career opportunities, particularly in growing product categories.

But corporate growth doesn’t automatically translate into individual compensation growth.

CMS Finalizes 2027 Medicare Payment Changes

Medicare’s newly finalized payment rules could influence where procedures are performed and how facilities evaluate medical technologies.

On November 4, CMS finalized its 2027 Hospital Outpatient Prospective Payment System and Ambulatory Surgical Center rules.

Three Numbers Worth Knowing

+2.3% | Outpatient and ASC Payments

Qualifying hospital outpatient departments and ambulatory surgery centers will receive a 2.3% payment-rate update.

637 | Procedures Removed From the Inpatient-Only List

CMS is continuing its multiyear effort to eliminate the inpatient-only list, expanding the procedures eligible for Medicare outpatient payment when clinically appropriate.

Up to -2.31% | Physician Fee Schedule Conversion Factors

The 2027 Physician Fee Schedule includes conversion-factor reductions of 1.83% or 2.31%, depending on participation in qualifying advanced alternative payment models.

Individual service reimbursement changes will vary.

Why It Matters for Reps

As more procedures become eligible for outpatient reimbursement, hospitals and physicians may reconsider where certain cases are performed.

That could create opportunities for manufacturers positioned to support outpatient and ASC workflows.

But payment updates don’t automatically improve facility profitability.

Reimbursement pressure, operating costs, and procedural efficiency will continue influencing purchasing decisions.

MDR Takeaway: Understanding reimbursement is becoming increasingly important for territory planning, account development, and competitive positioning.

INDUSTRY BRIEF

Three More Developments Worth Watching

Zimmer Biomet Reports $2.06B in Quarterly Revenue

Third-quarter revenue increased 3.1% on a reported basis, with management raising its full-year organic constant-currency growth outlook.

Medicare Expands Recognition of Software-Based Diagnostics

CMS finalized new outpatient payment classifications for certain Software as a Medical Service technologies, potentially supporting adoption of eligible diagnostic platforms.

ASC Economics Remain in Focus

Expanded outpatient eligibility and reimbursement changes could create new opportunities for device manufacturers, particularly those offering efficient procedural solutions and streamlined inventory management.

THE FINAL WORD

Loyalty can be valuable. But it shouldn’t be unconditional.

The latest MDR Compensation Index data shows that many long-tenured employees report substantial earnings. Meanwhile, Zimmer’s commercial restructuring demonstrates how quickly the economics of an established territory can change.

Your earning potential depends on more than your performance. It also depends on your compensation structure, territory opportunity, and the decisions being made around you.

Know your value. Understand your numbers. And make career decisions based on opportunity, not habit.
